Specialist Clinical Pharmacist / Care Coordination Pharmacist
Job Overview
Essex Partnership University NHS Foundation Trust (EPUT) is seeking an experienced and motivated Specialist Clinical Pharmacist / Care Coordination Pharmacist to join its pharmacy team in Rochford. This Band 7 NHS role offers an excellent opportunity to deliver high-quality clinical pharmacy services across inpatient sardar article and community settings.
Main Responsibilities
- Provide specialist clinical pharmacy services to patients in hospital and community settings
- Conduct medication reviews to improve patient safety and reduce hospital admissions
- Participate in multidisciplinary team (MDT) meetings and ward rounds
- Support safe transitions of care between hospital, community, and care homes
- Educate patients, carers, and healthcare professionals on medicines use
- Contribute to clinical audits, service development, and medicines optimisation
- Support elderly care, rehabilitation services, and complex patient needs
Essential Requirements
- Master’s Degree in Pharmacy (or equivalent qualification)
- Registered with the General Pharmaceutical Council (GPhC)
- Minimum of 2 years post-registration experience
- Postgraduate clinical pharmacy qualification (or working towards one)
- Strong communication and clinical decision-making skills
- Ability to work independently and as part of a multidisciplinary team
- Access to a personal vehicle for travel and on-call duties
