Job Overview
A permanent opportunity is available for a Specialist Clinical Pharmacist / Care Coordination Pharmacist to join the Essex Partnership University NHS Foundation Trust in Rochford. This full-time role focuses on medicines optimisation, patient-centred care, sardar article sardar article and supporting individuals to safely manage their medications.
Role Description
In this role, you will provide specialist clinical pharmacy services within a defined clinical or operational area. You will work closely with multidisciplinary teams and support patients both in clinical settings and in their own homes, helping to reduce unnecessary hospital admissions and improve health outcomes.
Key Responsibilities
- Provide specialist clinical pharmacy support to designated wards, units, or clinical teams
- Review and optimise patient medication, including home-based medicines reviews
- Support care coordination to help prevent avoidable hospital admissions
- Provide expert advice to ensure safe and effective use of medicines
- Contribute to the development of pharmacy and medicines management services
- Participate in clinical audits, service improvement, and quality initiatives
- Support education, training, and research activities where required
Skills & Experience Required
- Registered Pharmacist with the General Pharmaceutical Council (GPhC)
- Experience in clinical or hospital pharmacy practice
- Strong understanding of medicines optimisation and patient safety
- Excellent communication and teamwork skills
- Ability to work independently and within multidisciplinary teams
